When my son was newborn, I rolled a receiving blanket up and placed it under his bottom as long as it didn't pass the edges of the seat. It wasnt actually necessary but he sometimes slouched down when he slept and I wanted his head well above the neck line. I reach for this carrier more than I do my Ergo Night Sky. :)

Maybe because he can see me better and he feels closer to me that way. I use it either way. When I'm just resting and sitting down I let the carrier rests higher.

The hip strap is great it evens out the weight .. My dd is almost two and shes good size and i find no worries carring her arnd in this carrier.
